Over the next year, we will be making changes at our community health centers to improve how we care for your health care needs. We will be transforming your clinic into a Patient-Centered Medical Home.
Access will help you stay on track for all your medical screenings, exams, immunizations, and your scheduled appointments. We will assess your progress toward your medical goals and provide you with the most current evidence based care and self-management support so you will feel confident about managing your health at home. if you need a referral to a specialty provider we will help coordinate your care across multiple settings.
